One nurse will be presented with CancerSux.org's 2008 Thank God For Nurses That Care Award.

The Award Winner and the the individual who nominated them by essay will  receive a prize.  

Nomination Process

Submit an essay (maximum of 500 words) depecting your phenomenal nurse.

Each entry must include:

• your name 
• your address
• your telephone number
• your e-mail
• nurse’s name and credentials (if applicable)
• nurse's place of employment 
• nurse's telephone number (work or personal)
• nurse's e-mail [if available]

Entries without required information will be ineligible for the contest.

Nominations must be received no later than August 31, 2008.  Nominations must be complete and in compliance with instructions to be eligible for consideration.
